Enrolment Information

2025 enrolments will be accepted from Term 2 (15 April 2024) so please do not submit your enrolment before this date as they cannot be accepted.

​The Victorian Government Starting school webpage provides information for families on how to enrol their child in a Victorian government school and outlines the timeframes for submitting Foundation (Prep) enrolment applications for the 2025 school year.


What you need to do:

1. Check your school zone at https://www.findmyschool.vic.gov.au/

2. Book a Tour to learn more about our school and the enrolment application process.

3. Fill in an Application to Enrol in a Victorian Government School form below. This form must be filled in electronically, saved and sent back to the school email address middle.park.ps@education.vic.gov.au   We will send you an email to confirm we have received this form.

4. We will notify you of the outcome of your application middle of term 3.  If you receive an offer of placement, you should contact our school to accept the offer as soon as possible.  Upon acceptance, a full enrolment form will be sent to you and will need to be completed to finalise the application.

 

Please note:

 • Applications to Enrol submitted after July of the current year, will be processed by our school as they are received, and applications for children outside the MPPS zone will be assessed in line with the Department’s Placement Policy.


Applications to Enrol can be made by completing the form below.

Enrolment application forms can be sent to us by either:

  • Mailing it to MPPS, 194 Richardson Street, Middle Park 3206

 • Dropping it in to our school office

 • Emailing it to middle.park.ps@education.vic.gov.au

Our school zone

Our school zone is available on findmyschool.vic.gov.auFindmyschool.vic.gov.au hosts the most up-to-date information about school zones Victoria.

Students residing in this zone are guaranteed a place at our school, which is determined on the basis of your permanent residential address.

The Department provides guidance through the School Placement Policy to ensure that students have access to their designated neighbourhood school and the freedom to choose other schools, subject to facility limitations.

Application to Enrol forms

Applications to Enrol should be completed using the form below.

 • Enrolment Application form

​Along with this completed application to enrol form please provide the following documentation as listed below:

 • Birth Certificate.

 • Current Immunisation Statement. This is available via this link at the Department of Human Services or call 1800653 809.

 • Medical Action Plan (if applicable) Asthma/Anaphylaxis.

 • For Overseas students, a current visa.

To assist Middle Park Primary School in assessing your child’s eligibility for enrolment, please also include original or certified copies of:
1.    Rental agreements or unconditional contracts of sale plus
2.    A copy of two of the following:

 • Gas or electricity bill

 • Electoral enrolment confirmation

 • Other official documentation that demonstrates permanent residency at that address such as a driver’s license, health care card or council rates.

Documents should show the same address and parent’s/carer’s name as recorded on the school application to enrol form.

On request, you may also be required to complete a statutory declaration confirming the student is living in the address provided in the enrolment application and that the arrangement is genuine and intended to be permanent.

Note: Enrolment applications may not be successful if the requested documentation is not provided.

Changes to address after an enrolment offer is made
Subject to any changes to Department enrolment policy, if, after a placement offer is made to the student, the student’s permanent residence changes or it comes to light that the address provided on the application form was not the genuine permanent residence for the student, a school may only withdraw a placement offer in the following circumstances:

 • The placement offer made to the student expressly states that the offer may be withdrawn prior to the first day of attendance if the student’s permanent place of residence changes or the school becomes aware that the address provided on the application form was not the genuine permanent residence for the student; and the new address is not within the designated neighbourhood area for the school.

Prep Transition

Middle Park Primary School offers a positive transition to school with four transition sessions held throughout term 4, in the year before they begin school.

In order for us to best support a child’s transition to Prep, we also conduct a pre-Prep questionnaire to collect some preliminary details about the child’s interests, language development and any past or ongoing medical/behavioural matters.  Information provided greatly guides the school in the planning of class placements, strengthens our ability to begin building relationships with children and ensures we can individualise teaching and learning programs for our new cohort.  Information provided will be strictly confidential within the Prep and MPPS Leadership team.
